In a 2005 CRNA survey by recruitment firm LocumTenens.com, nearly 90% of respondents said they would stick to medicine if they could choose all over again.&lt;br /&gt;&lt;br /&gt;“We’re in touch with almost a third of the licensed U.S. nurse anesthetists, so these results don’t surprise me,” LocumTenens.com Vice President Lisa Kaeck said. “They enjoy the responsibility and challenge of the job and, of course, the compensation.”&lt;br /&gt;&lt;br /&gt;Generally, CRNAs provide anesthesia to patients under the supervision of surgeons, anesthesiologists and or physicians. A combination of the U.S. physician shortage and a rising number of in surgical procedures has increased demand for these practitioners, who generally earn less than anesthesiologists. “Many states face such a shortage of providers that they allow CRNAs to administer anesthesia without a physician supervising,” Kaeck notes.&lt;br /&gt;&lt;br /&gt;CRNAs aren’t complaining. Maintaining normothermia in the perioperative patient is a collaborative effort between the anesthesia provider, the surgeon, perioperative personnel and perianesthesia personnel.”&lt;br /&gt;&lt;br /&gt;The American Society of PeriAnesthesia Nurses (ASPAN)’s Evidence-Based Clinical Practice Guideline for the Promotion of Perioperative Normothermia states, “Perioperative hypothermia, defined as a core temperature below 36 degrees C, has adverse effects that range from patient thermal discomfort to increased morbidity and mortality. Even mild intraoperative hypothermia, a core temperature of 34 degrees C to 36 degrees C, has adverse consequences that are well documented ... The prevention of unplanned perioperative hypothermia and promotion of normothermia remains a national priority in the prevention of surgical site infection, and has been designated as a quality measure by the Surgical Care Improvement Project (SCIP).”&lt;br /&gt;&lt;br /&gt;According to the AST guideline, perioperative temperature management is imperative to positive surgical outcomes. Skin temperature, easily obtained during the perioperative period, is a function of external influences and thermoregulatory function of the body. Clinically available ‘near-core’ measures (e.g., oral, bladder, rectal, temporal artery, tympanic membrane [via infrared sensor], axilla) must be relied on to evaluate thermal balance across much of the perianesthesia/perioperative period. Typically, based on FGF, the following categories [2] can be listed:&lt;br /&gt;• High flow anesthesia uses a fresh gas flow, which is close to the patient’s minute ventilation (on average 3 to 6 L/min in a normal adult).&lt;br /&gt;• Low flow anesthesia uses a fresh gas flow of less than half the minute ventilation of the patient, which is most often less than 3.0 L/min on average in a normal adult. &lt;br /&gt;• In minimal flow anesthesia, the fresh gas flow is reduced to 0.5 L/min. &lt;br /&gt;• In closed system anesthesia, fresh gas flow is adapted to equal the patient’s need for oxygen and anesthetic agents. &lt;br /&gt;Why reduce fresh gas flows?&lt;br /&gt;Introduction of new volatile anesthetic agents with low solubility and low anesthetic potency has emphasized the need for reducing anesthetic agent consumption by lowering fresh gas flow. In addition to pure economic or ecological benefits, low flow may have a positive impact on the quality of patient care.&lt;br /&gt;Economic benefits&lt;br /&gt;Over 80 % of the anesthetic gases are wasted when fresh gas flow of 5.0 L/min is used. Several studies also prove that the use of low and minimal flow anesthesia techniques can dramatically reduce the annual costs of volatile anesthetics.
